RESUMO

In this study, a cholesteric chiral artificial receptor L5 was synthesized and a rapid method for the detection of iodide ions was established. The addition of L5 to Ethanol/Tris (V/V = 1/1, pH 7.0) solution changed the iodide ion solution from colorless to light yellow to achieve the naked-eye identification. The identification pattern was confirmed with the help of spectral titration, job test and anti-interference test, and it was found that L5 bound to iodide ion in a 1:1 coordination ratio. It was also confirmed that the hydrogen atoms of the amide group of MeO-PhCONH in L5 and the hydrogen atoms of -COOCH3 in L5 interacted with I- through hydrogen bonding force by 1H NMR titration, infrared spectroscopy, scanning electron microscopy and computer molecular simulation. The detection limit of L5 was 0.025 µmol/L, which could efficiently and conveniently detect iodide ions in food samples.

Objective:To investigate the effect of Portulacae Herba free granules on the degree of lens opacity and integrin β1 of lens in diabetic cataract rats. Methods:50 rats were randomly divided into control group, model group and the low, medium and high dose group of Portulacae Herba, 10 rats in each group. Except the control group, the diabetic cataract rat model was established by intraperitoneal injection of 1% STZ 60 mg/kg. The low, medium and high dose groups of Portulacae Herba were gavaged with Portulacae Herba 125, 250, 500 mg/kg, once a day for 12 weeks. The degree of lens opacity was observed with slit lamp. After 12 weeks, the lens was stained with HE. The expression of integrin β1 was detected by Western blot.Results:Compared with the model group, the opacification degree of lens in the low, medium and high dose groups of Portulacae Herba was significantly decreased ( P<0.05), the expression of integrin β1 (1.58 ± 0.14, 1.46 ± 0.24, 1.12 ± 0.19 vs. 1.76 ± 0.23) in rat lens was decreased ( P<0.05). Conclusion:For diabetic cataract rats, Portulacae Herba free granules can influence the expression of integrin β1 and inhibit the occurance of lens opacity.

Glioma is a common tumor in the central nervous system. Because the natural immunosuppression of tumor microenvironment is conducive to tumor growth, transformation and migration, the traditional treatment has little effect and is difficult to make a breakthrough. Glioma-associated microglia and macrophage (GAM), an important part of brain tumor microenvironment, plays a more and more key role in tumor progression and regulation of anti-tumor immune response. This article reviews the latest progress of the source, recruitment, polarization and the role in development of gliomas as well as potential therapeutic targets of gliomas.

BackgroundCoronavirus disease 2019 (COVID-19) has been declared as a threat to the global. Due to the lack of efficient treatments, indicators were urgently needed during the evolvement of disease to analyze the illness and prognosis, and prevent the aggravation of COVID-19. MethodsPatients general information, clinical type, all CK values and outcome were collected. CK value of all cases during disease course started from different initial time were analyzed. ResultsAll cases underwent 504 tests of CK since symptom onset and the median value was 51.7 (35.0-91.5) U/L. The first median value on the day 8 from exposure onset was 78.1 (69.1-85.8) U/L then showed an upward trend from the day 8 to the day 12 (reaching a peak of 279.3 U/L), finally showed a fluctuation decline after the day 12. The CK median value in critical cases reached the peak (625.5 U/L) on the transforming date, and then decreased rapidly to the normal range. Before death, the CK median value in dead cases firstly increased until the day -14 with a peak as 470.0 U/L, then decreased with fluctuation until day -2, and finally increased again on the day 0. ConclusionsCK reached its peak on the day when it became critical type, dynamic detection of CK can guide clinical judgment of prognosis. The increase of CK is a high risk factor of death. Severe cell damage 2 weeks before death might determines the outcome of the disease even if CK drops to the normal range afterward.

ObjectivesTo study the features of creatine-kinase (CK) in COVID-19 patients with different ages, clinical types and outcomes and quantify the relationship between CK value and clinical type. MethodsAll laboratory confirmed COVID-19 patients hospitalized in Xiangyang No.1 Peoples Hospital were included. Patients general information, clinical type, all CK values and outcome were collected. ResultsThe peak median value of CK in cases aged [≥] 71 years old (appeared at T2) was higher than that in cases aged [≤] 70 years old. There was statistical difference between the two groups (P=0.001). Similarly, the peak in critical cases (appeared at T2) was higher than moderate and severe types, and significant difference were existed among moderate, severe, and critical types (P=0.000). Moreover, the peak value in death group (appeared at T2) was higher than those in survival group. Significant difference was also found between them (P=0.000). According to the optimal scale regression model, the CK value (P=0.000) and age (P=0.000) were associated with the clinical type. ConclusionsDifference of the CK in different ages, clinical types, and outcomes were significant. The results of the optimal scale regression model are helpful to judge the clinical type of COVID-19 patients.

ObjectiveCoronavirus disease 2019 (COVID-19) is a serious infectious disease, which has caused great number of deaths and health problems worldwide. This study aims to examine the effects of airborne particulate matter (PM) pollution on COVID-19 across China. MethodsIn this study, we obtained confirmed cases of COVID-19, the data of airborne ambient PM with aerodynamic diameter [≤] 2.5 m (PM2.5) and [≤] 10 m (PM10), ambient temperature (AT), absolute humidity (AH) and migration scale index (MSI) in 72 cities of China (excluded Wuhan city) on a daily basis, each of which confirmed more than 50 cases from January 20th to March 2nd, 2020. We applied a two-stage analysis. Generalized additive models with quasi-Poisson distribution was first fitted to estimate city-specific effects of PM10 and PM2.5 on daily confirmed COVID-19 cases while controlling AT, AH and MSI. Then, we used meta-analysis to generate the pooled effect estimates from city-specific results. ResultsDuring the study period, there were a total of 24 939 COVID-19 cases, most of which were reported in Hubei Province. In our meta-analysis, we found each 10 g/m3 increase in concentration of PM2.5 and PM10 in single day lag (from lag 0 to lag 7 and lag 14) were positively associated with confirmed cases of COVID-19, not including PM10 at lag 5, lag 6 and lag 7, and PM2.5 at lag 5, lag 6. Similar trend was also found in different cumulative lag days (from lag 01 to lag 07 and lag 014). The effects of PM2.5 and PM10 on daily COVID-19 confirmed cases are statistically significant for three cumulative lag periods over 3, 7 and 14 days with the greatest effect over 14 days. The estimated RRs of which were 1.64 (95% CIs: 1.47, 1.82) and 1.47 (95% CIs: 1.34, 1.61) with each 10 g/m3 increase in concentrations of PM2.5 and PM10, respectively. In addition, we found that the effects of PM2.5 on daily confirmed cases were greater than PM10 in all included lag days. ConclusionsThis nationwide study suggests that airborne PM pollution likely increases the risk of getting COVID-19 in China.

ObjectMeteorological parameters are the important factors influencing the infectious diseases like severe acute respiratory syndrome (SARS). This study aims to explore the association between coronavirus disease (COVID-19) death and weather parameters. MethodsIn this study, we collected the daily death number of COVID-19, meteorological and air pollutant data from 20 January, 2020 to 29 February, 2020 in Wuhan, China. Then, the generalized additive model was applied to explore the impact of temperature, humidity and diurnal temperature range on daily mortality of COVID-19. ResultsThere were in total 2299 COVID-19 mortality counts in Wuhan. A positive association with COVID-19 mortality was observed for diurnal temperature range (r = 0.44), but negative association for relative humidity (r = -0.32). In addition, each 1 unit increase in diurnal temperature range was only associated with a 2.92% (95% CI: 0.61%, 5.28%) increase in COVID-19 mortality at lag 3. However, both per 1 unit increase of temperature and absolute humidity were related to the decreased COVID-19 mortality at lag 3 and lag 5, respectively. ConclusionIn summary, this study suggests the temperature variation and humidity may be important factors affecting the COVID-19 mortality.

Objective@#To explore the repair methods and effects of the complex wounds on hands after burns or trauma.@*Methods@#From January 2008 to December 2017, 45 patients (28 males and 17 females, aging 8 to 58 years) with severe hand injuries after burns or trauma combined with deep tissue exposure were admitted to our hospital. Two patients had whole-fingers degloving injuries, 27 patients had dorsal hand injuries, and 16 patients had palmar injuries. After debridement, the area of soft tissue defects was 7 cm×6 cm to 19 cm×12 cm combined with 0.5 cm×0.4 cm to 10.0 cm×4.0 cm of single deep tissue exposure. Different repairing methods were adopted according to the area and location of deep tissue exposure. Five patients with small area exposure were treated with artificial dermis+ vacuum sealing drainage (VSD)+ autogenous skin grafting. Thirty-eight patients with unilateral large area exposure on palm or dorsum were treated with segmented ligation of abdominal thin flaps (with area of 8 cm×7 cm to 15 cm×9 cm). Two patients with bilateral large area exposure in dorsal and palmar hands were treated with modified abdominal bag-shaped delayed thin flaps (with area of 12 cm×5 cm to 12 cm×9 cm and 12 cm×6 cm to 14 cm×9 cm). The donor sites were directly sutured or repaired with intermediate split-thickness skin or adjacent flap. The survival of grafts and flaps was observed, number of operations, wound healing time, and follow-up were recorded.@*Results@#(1) Among the patients receiving artificial dermis+ VSD+ autogenous skin grafting, the wounds of 3 patients were healed after 2 operations, and 2 patients had artificial dermis infection and lysis, and tendon necrosis, which were healed after 3 operations. The wound healing time of 5 patients was 14 to 33 days post injury. During the follow-up of 3 months, the affected hands were in good shape with soft texture and fewer scars, and functional evaluation of hand was good in 3 cases and modest in 2 cases. (2) The patients receiving segmented ligation of abdominal thin flaps all underwent 2 operations without flap necrosis. The wound healing time was 2 to 3 weeks post injury. Thirty-five patients underwent one to six-years′ follow-up, which showed that the flaps were in good shape and color with soft texture, and the functional evaluation of hand was excellent in 25 cases, good in 7 cases, and modest in 3 cases. Three patients were lost to follow-up. (3) In the 2 patients receiving modified abdominal bag-shaped delayed thin flaps, all flaps survived after 5 operations, the wounds were healed on post injury day 22 and 24 respectively, the shape and texture of the affected hands was good with no bloated appearance after separating fingers and revision. During the follow-up of 2 years, the functional evaluation of hand was good in 2 cases.@*Conclusions@#For the complex wounds on hands after burns or trauma, if the area of deep tissue exposure is small, artificial dermis+ VSD+ autogenous skin grafting should be adopted, which has good effects. If the area of unilateral deep tissue exposure is large, segmented ligation of abdominal thin flap should be adopted; if the area of bilateral deep tissue exposure is large, modified abdominal bag-shaped delayed thin flap should be adopted. These methods can reduce the number of operations, shorten wound healing time, and obtain good shape and function of hands.

Objective To evaluate the mitochondrial function of chondrocytes in Kashin-Beck disease (KBD) patients and its correlation with age.Methods Mitochondrial function was evaluated by analyzing the activities of respiratory chain enzyme complexes and citrate synthase (CS),intracellular adenosine triphosphate (ATP)contents, changes in mitochondrial membrane potential (ΔΨm),as well as the intracellular reactive oxygen species (ROS) contents.Correlation of mitodhondrial function and age was analyzed with linear regression.Results Activities of complexes Ⅱ (P = 0.001 ),Ⅲ (P = 0.005 )and Ⅳ (P = 0.005 )were reduced in KBD articular chondrocytes compared with cells from normal controls.However,the mitochondrial mass was increased in KBD samples. Cultured KBD chondrocytes had reduced cellular ATP level (P = 0.001 )and mitochondrial membrane potential (P =0.001),but increased citrate synthase activity (P =0.04)and intracellular ROS level (P <0.001).We found no correlation between mitochondrial function and donor age either in normal or in KBD chondrocytes. Conclusion Mitochondrial function is altered in articular chondrocytes of KBD,and no correlation was found between mitochondrial function and donor age.

Objective To investigate the efficacy and safety spinal ropivacaine and fentanyl-epidural anes-thesia for the first stage of preclinical of childbirth.Methods 78 cases of full-term maternal were divided into the three groups randomly by using digital representation,each group had 26 cases.A group did not implement analgesia, B group was given analgesia in the first stage of incubation(ropivacaine and fentanyl epidural anesthesia),C group was given labor analgesia on the first line of the active phase.The labor time,mode of delivery,Apgar score,pain VAS score and the occurrence of adverse reactions of three groups were observed.Results The VAS pain scores of B group were (5.01 ±0.61)points,(2.30 ±0.32) points and (1.65 ±0.21) points after 5,10,30min of analgesia, which were significantly lower than those of before(t=6.59,9.62,10.73,all P0.05) at each time point;The first stage of labor of B group and C group were (327.43 ±31.02)min and (335.62 ±30.18)min,respectively,which were significantly decreased compared with A group(t=7.21,6.90,all P0.05).Conclusion Ropivacaine and fentanyl in epidural anesthesia had a preferable effect for the first stage of preclinical and active period of childbirth which can effectively reduce maternal labor pain and improve the natural birth rate and has a fewer adverse reactions.

Objective To summarize the etiologic factors and evaluate the clinical outcome of reoperations on 119 limbs of recurrent varicose veins.Methods Retrospective analysis was made on etiologic factors of 119 limbs (113 patients) of recurrent varicose veins admitted to our hospital from January 2007 to June 2013.All patients underwent color duplex ultrasonography and anterograde venography of the lower extremity and were treated by reoperations.Results In the 119 limbs,102 limbs (85.7%) had residual main great saphenous veins or tributaries,97 limbs (81.5%) had incompetent perforator veins,23 limbs (19.3%) had neovascularization,9 limbs (7.6%) had incompetent small saphenous veins,21 limbs (17.6%) had incompetent femoral veins,6 limbs (5.0%) had iliac vein compression syndrome,and Budd Chiari syndrome was found as the cause of recurrence in 1 limb (0.8%).Postoperative patients were followed-up for 6-72 m (32-± 7 m).The cure rate of varicose veins were 100%.There was not recurrence of varicose veins,postoperative VCSS was 0-5 (1.2 ±0.5) vs preoperative 1-17 (6.2 ±2.5)(P < 0.01).Conclusions Residual and incompetent great saphenous veins or tributaries and incompetent perforator veins were the main etiologic factors of postoperative recurrent varicose veins.Therapeutic principle for recurrent varicose veins is to eliminating the reverse flow of superficial vein system and ligate incompetent perforator veins.

RESUMO

This study investigates the production of organic fertilizer using Anaerobic Digestate (as a nutrient source) and limestone powder as the raw materials. A two-level factorial experimental design was used to determine the influence of process variables on the nutrient homogeneity within the granules. Increasing the liquid-to-solid ratio during granulation resulted in increased granule nutrient homogeneity. Increasing the processing time and the impeller speed were also found to increase the nutrient homogeneity. In terms of nutrients release into deionized water, the granules effectively released both potassium and phosphate into solution.

Objective To investigate the changes of amplitude of low frequency fluctuation (ALFF) of the resting state fMRI in cognitively impaired Parkinson' s disease patients and discuss its underling neurophysiological mechanism.Methods Blood oxygen level-dependent low-frequency amplitude (ALFF) in resting-state functional magnetic resonance imaging were calculated in 16 healthy controls(HC) and 29 idiopathic Parkinson's disease patients (16 of which were patients with cognitive normal,PDCN and 13 with cognitive impairment,PDCI).The brain regions showing increased and decreased ALFF in patients were demonstrated by comparing normal subjects with 2-sample t-test with threshold of P< 0.05 and the analysis of the relationship between the different regions of the brain activity and cognitive function tests scores were also analyzed.Results Compared with PDCN,the PDCI patients showed decreased activity in the caudate nucleus (-3,9,12),occipital lobe (0,-78,-15) and medial temporal lobe (42,9,-27) and increased activity in the superior frontal gyrus (9,63,24).PDCI patients showed increased activity mainly in the precuneus and inferior parietal lobules compared with controls.Additionally,the regions with ALFF changes had significant correlations with the cognitive performance of patients as measured by Montreal cognitive test(Beijing Version) and neuropsychological tests (including memory,attention,visuospatial functions and executive function).Conclusion The results demonstrate that there is a specific pattern of intrinsic activity in PDCI providing insights into neurophysiological mechanisms of the Parkinson's disease dementia.

Objective To explore the necessity and function of the sustentaculum tali screw placement for the treatment of Sanders type Ⅱ calcaneal fracture.Methods The data of Dicom that CT scan with bone was input into Mimics 12.0 software and Ansys 13.0 software for construction of calcaneus three-dimensional finite element model.Then,this model was imported into Solidworks 2010 software.Type Ⅱ calcaneal fracture model was established according to Sanders type cutting of calcaneus.Geometric parameters of AO calcaneal plate and screws were imported to the Solidworks 2010 software.Two internal fixation simulations were established based on the calcaneal model.In one model,the steel was placed on the outside with bone and the cancellous' bone screws were infiltrated respectively into the vertical bone since two screw holes which under the articular surface behind the steel plate,two screw holes which rear of the steel plate,a screw hole of the below calcaneus,two screw holes which in front of steel plate.In another model,a cortical bone screw was infiltrated into the sustentaculum tali through the bottom screw hole of articular surface of the steel plate.The two internal fixation models were loaded under the same condition,and non linear finite element analysis was carried out.The stress distribution in the two internal fixation models was calculated respectively.Results The maximum principal stress focused on the cortical bone of sustentaculum tali in both of the models under the same condition of loading.Bone joint displacement,maximum principal stress of calcaneal and internal fixation system in the model with sustentaculum screw fixation were smaller than that in the model without sustentaculum screw fixation.The stress in the model with sustentaculum screw fixation was more dispersed.Conclusion The placement of sustentaculum tali screw is essential for fixation of type Ⅱ calcaneal fracture to achieve the biomechanical stability and can be used in clinical.

Objective To explore the effect of three-dimensional conformal radiation dose fractionation treatment of advanced non-small cell lung cancer(NSCLC).Methods 75 NSCLC patients were treated with hypofractionated 3DCRT( observation group),while 73 cases were treated with conventional fractionated radiotherapy (control group).The efficacy and adverse reactions were observed;Survival after treatment were followed up in 1,2,3 years.Lung function was detected before and after radiotherapy treatment,including FVC,FEV1 and CLCO.Results The total effective cases of control group were 47 cases patients ( 64.4% ),observation group's was 60 cases ( 80.0% ),total effective rate had statistically significant difference ( x2 =4.50,P < 0.05 ).Survival of control group after treatment in 1,2,3year were 50.7%,24.7%,8.2%,the median survival was 13 months,observation group's were 73.3%,45.3%,20.0%,and 19 months.The survival of these two groups was statistically different (x2 =8.07,6,94,4.22,all P < 0.05 ).The patients blood system side effects of observation group were significantly lower than the control group ( x2 =4.73,P <0.05 ) ;acute radiation pneumonia,esophagitis of these two groups had no significant difference in the incidence( P > 0.05 ).Conclusion Hypofractionated 3DCRT treatment of advanced NSCLC had good effect,and its adverse reactions was low,and it was worthy of clinical application.

Objective To investigate the clinical effect of hyper-fractionated three-dimensional conformal radiotherapy (3DCRT) combined with transcatheter arterial chemeembolization (TACE) for heating patients with inoperable primary liver cancer.Methods One hundred and two patients with inoperable advanced primary liver cancer were divided by random digits table method into control group with 49 cases treated with TACE and observation group with 53 cases treated with hyper-fractionated 3DCRT combined with TACE.The short-term efficacy,adverse reaction and survival rate of the 1st,2nd and 3rd year and serum alpha-fetoprotein (AFP) expression after treatment in two groups were observed.Results The complete remission rate,progress rate and total efficiency were 17.0% (9/53),11.3% (6/53),73.6% ( 39/53 ) in observation group,2.0%( 1/49 ),28.6% (14/49),51.0%(25/49 ) in control group.There were significant differences between two groups (P < 0.05).There was no statistical significance in adverse reactions between two groups ( P > 0.05 ).The survival rates of the 1 st,2nd and 3rd year in control group were 51.0% ( 25/49 ),28.6%(14/49),14.3%(7/49),the median survival time was 15 months;while those in observation groupwere 71.7%(38/53),52.8%(28/53),39.6%(21/53) and 20 months.The survival rates of the 2nd and 3rd year in observation group were significantly higher than those in control group respectively (P <0.05 or <0.01 ).The rate of patients with no obvious serum AFP protein change between observation group [ 9.8%(4/41) ] and control group [ 31.6%(12/38) ] had statistical significance (P < 0.05).Conclusion Hyper-fractionated 3DCRT combined with TACE for treating patients with inoperable primary liver cancer has good clinical efficacy,it is worthy of clinical practice.

Objective To explore the causes of hepatic perfusion disorders (HPD), and to analysis the CT features of this phenomenon. Methods Nine experimental pigs were randomly divided into three groups. In Group A, B and C, intrahepatic portal, arterial branches and hepatic vein were selectively embolized, respectively. Then all animals underwent contrast enhanced CT scans immediately and after one week. Results On CT images immediately after intervention, HPD were found in all animals. Multiple transient wedge-shaped hyperattenuation represented during the hepatic arterial phase and isoattenuating areas during the portal venous phase. In group A, the site of HPD was in correspondence with the area of embolization,whereas in group B, the embolized area displayed hypoattenuation and non-embolized area displayed hyerattenuation during arterial phase. In group C, the site of HPD was correspondence with the area of embolization in two pigs, but in one pig, the region of HPD was larger than the region of embolization. After one week, HPD disappeared due to sponges absorption and the vessels reopening in group A and B. In group C which hepatic vein was coagulated by laser, HPD remained partially. Conclusion Intrahepatic vascular occlusions of portal, hepatic arterial and hepatic venous branches are the main factors that cause HPD.

RESUMO

Objective To study the relationship between the level of plasma soluble P-selectin and pediatric migraine. Methods ABC-ELISA method was used to detect the soluble P-selectin levels in migraine patients (migraine group), migraine patients under control (control group) and healthy children (healthy control group). Transcranial Doppler ultrasound (TCD) and the platelet count were performed. Correlation of P-selectin and platelet count was analyzed. Results Plasma soluble P-selectin level in migraine group was significantly higher than that in the control group and the healthy control group (both P 0.05). TCD showed cerebral vasospasm more frequent in the migraine group than the control group, with significant difference (P < 0.05). Blood examination revealed that platelet count of migraine group is higher than that of the control group and the healthy control group, with significant differences (P < 0.05). When migraine was under control, the level of plasma soluble P-selectin reduced to normal and the frequncy of cerebral vasospasm and platelet counts were also within normal range. Plasma soluble P-selectin level and platelet count were positively correlated (r = 0.996, P < 0.01). Conclusions Plasma soluble P-selectin increased in migraine children, the incidence of cerebral vasospasm was significantly increased. Plasma soluble P-selectin level and platelet count were positively correlated. Plasma soluble P-selectin can be used as one of the indicators of diagnosis, treatment and efficacy evaluation in pediatric migraine.

RESUMO

Objective:To investigate the use and adverse drug reaction of aminoglycoside antibiotics(AmAn) in our hospital during 2005-2008,and to valuate its rationality for reducing its adverse drug reaction.Methods:The data about AmAn's consumption quantity and consumption sum in our hospital were analyzed with the DDD method.Results:Etimicin dominated the first place among AmAn in our hospital,and replaced Gentamicin.The quantities of Netilmicin,Isepamicin and amikacin were stable.In the same period,the main adverse drug reactions were allergic-like reactions,skin rash,ototoxicity and local numbness.Conclusion:Careful selection according to etiology,genetics testing,stronger monitoring of blood concentration and rational dosage treatment will provide a good prospect in the use of AmAn for the multifaceted gram-negative bacteria infection.
